Payment Policy
All payments are due at the time of service. Payment may be made by cash, personal check, money order, Visa, MasterCard, Discover, American Express or CareCredit.

Cancellation Policy
Kindly give a 24- hour notice to avoid a cancellation fee. We understand unexpected circumstances may arise and in this event we do have a voicemail system in which you are able to leave a message in case you are unable to reach the office staff.
Treatment of Minors
Patients under the age of 18 must be accompanied by a parent or a legal guardian, or have written permission for treatment.
Running On Time
We know your schedule is busy and that your time is valuable. We strive to keep our wait times reasonable, please also keep in mind we do accommodate urgent cases that require extensive care which may delay our schedule. Please also remember we are running several different schedules. If someone who arrived after you is called before you, they might be having a quick nurse-only injection or seeing a different provider.
